Gastric cancer is known to be highly prevalent among all the primary gastrointestinal malignancies globally, it is also considered to be the third most deadly cancer. Often it has been noticed that the cases of gastric cancer are prevalent based on geographical areas like Latin America, East/Central Asia, and Eastern Europe are more likely to be affected by this as compared to other parts of the world. Even though we have a lot of advancements when it comes to gastric cancer like early detection, improved medications, etc. Studies showed that still nearly two-third of people having higher risks are being affected by the progression of this disease also known as recurrence. Due to this major reason, Adjuvant or perioperative chemotherapies are being majorly investigated to find an ultimate cure for this disease. S-1 is an oral chemotherapeutic substance seen to have yielded positive output in Asia, S-1 is composed of tegafur, oteracil, and gimeracil. S-1 is seen to be low toxic i.e., without any serious side effects, is also well-tolerated, and is easily available for outpatient settings, currently, it has been approved by Europe and Asia but not the US. Through this study, the scientist tried to show a comparison of platinum-based doublets under D2 gastrectomy as they have not been reported much in real-world experience.

Brief About the Study

For this study scientists took 433 Han Chinese patients who had gastric cancer (perioperative chemotherapies) from the year Jan 2008 to Oct 2018 an observation was being made on patients who received surgery with close observation (OBC), S-1 and Platinum-based doublet (P) the number of patients in this was 221, 67 and 145 respectively. To compare the recurrence-free (RFS )and overall survivals (OS) in patients who were locally advanced in the disease, a propensity score matching was used. Adverse reactions, side effects, dosages that should be administered were also discussed briefly in this study.

Results

A total of 647 people were screened out of which only 433 were eligible for this study a follow-up time of 51.9 months was given, it was noticed that patients who were given S-1 (adjuvant S-1 monotherapy) showed an intermediate level of survival when compared to OBS and P the comparison was done like S-1 vs. OBS, 24.4/40.2 vs. 20.7/27.0 months, HR = 0.62/0.55, p = 0.041/0.024; RFS/OS: S-1 vs. P, 20.9/35.8 vs. 31.2/50.5 months, HR = 1.76/2.14, p = 0.021/0.008. With a dose intensity of 73.6%, S-1 was well tolerated with a median duration of 8.3 months and showed fewer adverse effects or side-effects as compared to others, especially P, a prominent survival difference was noticed in patients with N2–3 diseases. The selection of the S-1 monotherapy was made by the physicians according to the disease stage, age, lymph node stage, and also the serum carcinoembryonic antigen.

Conclusion

It was noted through this study adjuvant S-1 monotherapy has brought a revolution when it comes to Stage II and III gastric cancer especially in people who have received D2 gastrectomy especially in Asia. This study showed a thorough clinical stage of various treatments provided and also showed a comparison between them i.e., S-1 versus close observation and conventional platinum-based chemotherapies.
