Osteoarthritis (OA) is the most prevalent kind of arthritis and one of the major causes of decreased mobility and persistent pain, impacting over half of the world’s population aged 65 and up. The most common articulating joint disease in humans is osteoarthritis (OA). It has been noted that hand, knee and hip OA are the most common forms of the disease, and the knee is the primary area of focus in the majority of research. Reducing pain and slowing joint deterioration development in individuals with knee OA (KOA) is still a difficult challenge. Early diagnosis and assessment of KOA prognostic variables are critical for establishing management and therapies aimed at preventing permanent knee joint degeneration and arthroplasty. As a result, one of the main aims of imaging biomarkers is to identify individuals who are at a high risk of developing KOA.
The vertical and horizontal trabeculae of a designated bone area of interest are examined in TBTA (Trabecular bone texture analysis). TBTA has been discovered as a biomarker for trabecular bone alterations caused by KOA. As a result, a complete examination is required to have a better grasp of this unfamiliar image analysis approach in the field of KOA research.
In a review recently published in the journal Arthritis Research & Therapy, researchers used the PubMed electronic database to conduct literature searches. Traditional and fractal image analysis of trabecular bone texture (TBT) on knee radiographs were discovered in studies published between June 1991 and March 2020.
The Study
A total of 219 papers were found. After sifting through titles and abstracts, 39 studies were found to be eligible, and they were divided into six categories: cross-sectional evaluation of osteoarthritis and non-osteoarthritis knees, understanding of bone microarchitecture, prediction of KOA progression, total knee arthroplasty and treatment response, and KOA incidence. The importance of TBTA as a possible bioimaging marker in the prediction of KOA occurrence and development has been highlighted in several studies. Only a few studies have looked at the relationship between TBTA and OA treatment responses as well as the likelihood of knee joint replacement.
The future of KOA treatment
The biological plausibility of TBTA in KOA has previously been demonstrated. TBT has a consistent relationship with key KOA outcomes including KOA radiographic incidence and progression, according to the review. TBTA might offer indicators for clinical trial enrichment, allowing for better screening of KOA progressors. Significant progress has been made toward a completely automated KOA evaluation.
