Impact of sex in diabetes and hemorrhagic stroke patients

Patients’ adept to ischemic stroke and neurovascular disease, diabetes has been a major risk in their lives. Statistics reveal that this condition is common in 20-33% of the human population. Studies show that diabetic patients have a devastating long-term vascular prognosis and higher mortality rate after suffering a hemorrhagic stroke. The impact of sex in diabetic and hemorrhagic stroke patients plays a crucial role in the incidence and outcomes of the sufferings. As, amongst diabetic women, the risk of intracerebral hemorrhage (ICH) stands to be significantly high, as compared to subarachnoid hemorrhage (SAH).

Analogy Based on The Study in Spain.

A population-based study in Spain reveals those women suffering from type 2 diabetes are least likely to undergo the therapeutic procedures when compared to men. Also, the study depicted that there was a difference in the data regarding the impact of sex amongst the non-diabetic patients, suffering from hemorrhagic stroke. Long-term analysis hardly states any difference within the patients suffering from type two diabetes were compared based on sex for hemorrhagic stroke mortality. For conducting the study, the propensity score matching strategy was used by collecting the outcome data from the hospitals. Further, the variable associated with in-hospital mortality for type 2 diabetes patients were identified according to sex.

The Conducted Study

A study was based on several parameters like retrospective, epidemiological, and observational strategies. It was conducted using the discharge reports of the hospital through Spanish National Hospital Discharge Database (SNHDD). The study used data records of two years- from 2016 to 2018. Patients belonging to the age group of 35 years or above were selected for hemorrhagic stroke diagnosis. Along with the analysis of patients (both male and female), with and without type 2 diabetes for the prediction of results, the use of decompressive craniectomy was also studied. Analysis was done considering and comparing both ages as well as sex. Moreover, conditions such as hypertension, lipid metabolism disorder, alcohol abuse, etc., followed by antiplatelet count were also taken into consideration during the analysis. Comparative sub-populations were created based on the propensity score matching method.

Results:

A total of 56,400 patients, were analyzed based on which 11,915 patients were diagnosed with type 2 diabetes. The results revealed that the rate of type 2 diabetes was higher amongst men than among women. Further, it was analyzed that the rate of hemorrhagic stroke was higher in patients with type 2 diabetes. Results stated that based on sex, hemorrhagic stroke was 21% higher amongst males with type 2 diabetes as compared to males without type 2 diabetes. The rate of ICH was observed to be higher in men having type 2 diabetes, whereas, in the case of SAH, the rate was higher in men without diabetes. The antiplatelet count was more prominent amongst men possessing type 2 diabetes (in Table 1).

Conclusory Remarks:

The analysis thus proves the impact of sex-based on population study; the rate of hemorrhagic stroke was higher in patients suffering from type 2 diabetes in a maximum of the age groups. Finally, type 2 diabetes is linked to increased hemorrhagic stroke incidence and less utilization of decompressive craniectomy in both sexes, even if only in females with higher In-Hospital Mortality. Higher incidence rates, greater decompressive craniectomy, and lower In-hospital mortality among men than in women are present in the sex differences in type 2 diabetes patients experiencing a hemorrhagic stroke. In men and women with or without type 2 diabetes, the use of Oral Anticoagulants and anti-platelet medicines was linked to an increase in-hospital mortality.
