An important poor prognostic factor of prostate cancer (PC), visceral metastasis (VM), is not commonly observed in castration sensitive progression but continuously seen after castration-resistant progression. Still, the site, timing of appearance, and incidence of VM in castration-resistant patients have not yet been fully explained.
At Kanazawa University Hospital, demographic, surgical, pathological, and follow-up data of PC patients have been treated between January 2000 and December 2016 were retrospectively examine using their medical charts. From these figures, risk factors of visceral metastasis and survival of patients with it were explained.
21 patients (1.5%) had visceral metastasis at diagnosis out of 1364 patients. 55 experienced the emergence of a new VM during treatment course out of 179 (13.1%) castration-resistant patients. Incidence of new VM, especially non-lung, like liver and adrenal metastases, grow significantly in proportion with the number of authorised treatments. Multivariate analysis showed that T stage, M stage, age, and treatment report with androgen receptor (AR) signalling-targeted agents and/or taxanes significantly grow the risk of VM. Compared with the group with VM at diagnosis, survival after diagnosis of VM following treatment was shorter.
In spite of the fact that sequential use of new AR signalling-targeted agents and taxanes for castration-resistant prostate cancer (CRPC) is a standard treatment plan, it frequently results in the development of VM. Explaining the mechanisms of VM are key to enhance survival in patients with CRPC.
