Elevation of serum IgA and antiphospholipid-IgA antibodies production in Severe COVID-19

Based on what we know so far about the complications of COVID-19, it frequently involves complications that bear similarities to autoimmune disorders. Up to now, there are little reports on possible IgA mediated autoimmune responses. A study was conducted to determine the association of COVID-19 with vigorous total IgA responses. It also aimed to determine the association of IgA antibodies with complications of severe illness. The most frequent event involves in severe COVID-19 is the thrombotic event which resembles the hypercoagulation of antiphospholipid syndrome (APS).

In this study, clinical data and antiphospholipid antibodies (aPL) production from 64 patients with COVID-19 were compared and reviewed from three independent tertiary hospitals. Clinical records obtained from the 64 patients with COVID-19 were divided into three categories: one with mild illness (mCOVID) (41%), another with a discovery cohort with severe illness (sdCOVID) (22%), and a confirmation cohort with severe illness (scCOVID) (38%). The total production of IgA, IgG, and aPL was measured. 

Results revealed a significant association of severe illness with IgA production (sdCOVID, P=0.01; scCOVID, p-value<0.001) but not with total IgG production. Both cohorts with severe illness were significantly associated with elevated anti-Cardiolipin IgA, anti-Cardiolipin IgM, and anti-Beta2 Glycoprotein-1 IgA. Thus, a  strong association was found between higher production of IgA and aPL with severe illness. This suggests triggering of a strong antiviral IgA response in bronchial mucosa, induces systemic autoimmunity.
