Severity and death from Coronavirus disease 2019 (Covid-19) are associated with the older population; testing such vaccine candidates is important to prevent infection with severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2).

A phase 1, open-label trial with an escalated dose of messenger RNA vaccine, mRNA-1273, which encodes the stabilized prefusion SARS-CoV-2 spike protein (S-2P) was conducted on 40 older adults (56 to >71 years). On receiving two doses of 25 μg or 100 μg of the vaccine for 28 days apart, adverse effects were mainly mild or moderate.

 Shortly after the first immunization, binding-antibody responses rapidly increased. Higher binding- and neutralizing-antibody titers were induced in 100-μg dose than the 25-μg dose, thus, supporting the use of 100-μg dose in a phase 3 vaccine trial. Funded by the National Institute of Allergy and Infectious Diseases, the vaccine also elicited a strong CD4 cytokine response involving type 1 helper T cells.
